How Can Medicare Advantage Plans in Hawaiʻi Have a $0 Monthly Premium?

One of the questions we hear most often from people exploring Medicare is:


“If healthcare is so expensive, how can some Medicare Advantage plans have a $0 monthly premium?”


It’s a fair question.


At first glance, it almost sounds too good to be true.


Many people wonder whether there’s a catch or if the plan is somehow “free.”


The answer is actually much simpler than most people think.


A $0 monthly premium doesn’t mean healthcare is free. It simply means you don’t pay an additional monthly premium to the insurance company for that Medicare Advantage plan. You must still continue paying your Medicare Part B premium (unless you qualify for a program or benefit that helps pay it), and you may still have copayments, coinsurance, or other out-of-pocket costs depending on the services you receive and the plan you choose.

Medicare Pays Private Insurance Companies to Provide Your Coverage


When you enroll in Original Medicare, the federal government pays healthcare providers directly for the Medicare-covered services you receive.


With Medicare Advantage, the process works differently.


Instead of paying doctors and hospitals directly through Original Medicare, the federal government pays private insurance companies a set amount each month for each person enrolled in one of their Medicare Advantage plans.


Those insurance companies are then responsible for providing all of your Medicare-covered benefits.

Why Can Some Plans Charge a $0 Monthly Premium?


Because Medicare already provides funding to the insurance company, some Medicare Advantage plans choose not to charge members an additional monthly plan premium.


Instead, they may:

  • Receive funding from Medicare to administer your benefits.

  • Manage healthcare costs through provider networks and coordinated care.

  • Compete for members by offering attractive premiums and supplemental benefits.

  • Use quality incentive payments when they qualify.


Every insurance company develops its own strategy.


Some charge an additional monthly premium.


Others choose to offer a $0 plan premium while structuring costs differently through copayments, provider networks, and other plan features.


Does $0 Mean “Free”?


No.


This is probably the biggest misconception about Medicare Advantage.


A $0 monthly premium simply means you are not paying an additional monthly premium for the Medicare Advantage plan itself.


You will generally still need to:

  • Continue paying your Medicare Part B premium.

  • Pay copayments or coinsurance when you receive certain healthcare services.

  • Pay prescription drug costs if your plan requires them.

  • Pay for services that may not be covered under your plan.


Every Medicare Advantage plan has its own schedule of benefits and cost-sharing.


That’s why comparing plans involves much more than simply looking at the monthly premium.


Why Do Insurance Companies Want to Offer $0 Premium Plans?


Like many businesses, insurance companies compete for customers.


Offering a $0 monthly premium is one way to make a plan more attractive to Medicare beneficiaries.


However, that doesn’t necessarily mean the plan costs the insurance company less to operate.


Instead, each company balances many factors, including:

  • Provider contracts

  • Healthcare utilization

  • Prescription drug costs

  • Administrative expenses

  • Government payments

  • Member health needs


Every company’s approach is different.


Medicare Rewards High-Quality Plans


Another reason some Medicare Advantage plans can offer competitive benefits is Medicare’s Star Rating System.


Each year, Medicare evaluates Medicare Advantage plans based on factors such as:

Should You Choose a Plan Based Only on the Premium?


In our opinion…


No.


While a $0 monthly premium may sound appealing, it should never be the only factor you consider.


Instead, ask questions like:

  • Are my doctors in the plan’s network?

  • Are my prescription medications covered?

  • What are my copays for the services I use most?

  • What is the annual maximum out-of-pocket limit?

  • Do the extra benefits matter to me?

  • Will this plan continue to meet my needs if my health changes?


Sometimes a $0 premium plan is an excellent fit.


Other times, another plan with a monthly premium may provide greater value based on your personal healthcare needs.


The goal isn’t to find the cheapest plan.


The goal is to find the right plan.


What About Hawaiʻi?


Hawaiʻi has a unique healthcare environment.


Provider networks, hospitals, specialists, and available Medicare Advantage plans can vary by island.


That’s why two people living in different parts of Hawaiʻi—or even two neighbors with different doctors or prescriptions—may choose completely different Medicare plans.


Choosing the right Medicare Advantage plan involves much more than comparing premiums.


It’s about finding a plan that works well with your doctors, medications, lifestyle, and budget.
